Default Withdrew money from annuity

Last summer I withdrew the balance of my pre-tax annuity(AIG) without penalty (just turned 60). I paid fed and state taxes at that time prior to getting my money. What do I need to do for this coming up tax filing time? I plan to use form 1040.
Any assistance will be appreciated.
