Fifty50 is happy to provide samples of our products for use at your diabetes education programs and activities. Because of the large number of requests we receive, we ask that you please comply with our policies so we can continue to serve as many educators and their clients as possible.
Send your request at least four weeks in advance of the date needed. (This is critical to make sure your samples arrive on time.)
Submit only one request per year. (A sample request form for your use the following year can be downloaded here.)
Choose the one option that best fits the needs of your planned program or activity.
