I have worked in the food industry and also have a few close friends/family members that work at some places you mentioned. Any time you eat in a restaurant, your rolling the dice really, because you can't see your food being prepared and the quality of your food is reliant on the person preparing it. (Ever see the movie "Waiting"?)
Most fast food restaurants do not use mechanically separated meat in their nuggets, and when mechanically separated meat is used it must be listed as such. Most nuggets with mechanically separated meat comes in the form of the nuggets you buy at the grocery store to prepare at home. Plus...it IS just chicken, as revolting as some people may find the process.
Wendy's does use leftover hamburgers in their chili, but the hamburgers should not have sat out all day according to Wendy's guidelines and food service guidelines.
From my own experience at jobs in my 20's, your dead on with the ice from soda fountains. Although I have never seen anything but clean ice go in to them, I never saw them cleaned either. I don't really clean my ice trays at home all that often, but in this case you have to trust the employee, and I tend to not in that situation.
It never fails to amaze me how many people don't understand basic maths / common sense. This becomes obvious when you consider how many people are putting money away in a 2% interest savings account instead of using it to make overpayments on a mortgage being charged at 4% interest!
Pssst...Wendy's chili is made with day-old burgers. No "probably" or "maybe" about it. But even as a former employee, it's about the only thing I buy from there. I appreciate the frugality of it.
What I wouldn't eat? Eggs from any fast food joint. They're from a milk carton (the infamous "breakers", which are already pretty scary), they cook them and hold them in a steam cabinet for disturbing amounts of time, and I've seen them turn grey and still be served. Ick, ick, ick!!!
I love the basic message in the article's title, that many people are to blame for their financial worries. It has actually taken me years to understand it myself, and I think I have gotten my finances under control only a few years ago. Now, I have a huge loan from my education to repay, because in the past I chose not to work a lot during education and lead a party life.
The funny thing is, that only from the point I realized that it was me who put myself in this situation, I got my finances under control. From that point on, I was able to start living frugally, and spend my money on things I really needed, or which truly increase the quality of my life. And actually I started becoming more satisfied with less.

They said, they have been in business for 10 years, but some how I do not believe them. They said, they sent me several emails requesting required documents. But I received only one email from them and it was sent 7 minutes after the hour long call was ended. Whoever they are or were, they need to regulated. The whole industry needs to be regulated. UGGGH.
I used to get it once in awhile, usually late at night when I had gone straight from work to a rehearsal and never got the chance to eat more than a snack. Now, I really try to plan and bring something I can eat, and I succeed perhaps 98% of the time!
Prior to having my son I ate fast food a couple times a week. And then when he started eating solids I became very aware of what is in our food. So I dropped down to once a week and the less I eat it the better I feel. I find myself encouraging us to eat at home rather than eat on the run. Or to finish our errands so that we can be home in time for lunch or dinner.
I try not to eat out very often (fast food included), but sometimes it feels nice to just relax and not worry about it. I take a night off from cooking about once a week. When I do go out I like to hit up some of the local restaurants (they usually have better discounts/frequent buyer cards/etc.). I am a big fan of the McDonald's Monopoly game, though.
I will admit, I eat more than I should. I also prepare homemade meals, but once or twice a week while I am at work I will go through the drive thru. I have coupons in my purse so I usually don't spend more than $3. I know the politically correct answer is "never! I eat homemade organic food all the time and it's so much more delicious!", but I will honestly say I like the taste of fast food! And obviously I am not the only one because these restaurants make billions of dollars.
Trader Joe's in uptown Manhattan is a madhouse - poor store layout, and NO TWO BUCK CHUCK!! (Some NY law prohibits them from selling it). Also, they are frequently out of tofu, and don't carry a vegan veggie burger, so it's a pretty useless store for me. I do love going to Trader Joe's in MA where my mom lives - nice layout and they sell TWO BUCK CHUCK which is probably the best thing ever to happen to wine! (Jk, but it is still awesome!) Downtown, they have a separate wine store, but it's not a convenient location for me.
I am a single Manhattanite with a small kitchen who is bike commuter and Costco member (weird, right?) I can usually fit everything I need in my three bike baskets. I usually stop in once a week, sometimes twice. They have tofu at great prices, awesome vegan veggie burgers, and tons of other organic and vegan stuff like stiffed grape leaves. I do supplement my Costco shopping with trips to Whole Foods to pick up my "weird" stuff (vital wheat gluten flour, liquid stevia, etc.). I tend to eat the same thing for weeks at a time (always have), so Costco is a great solution for my mono-eating tendencies. Also, the membership fee is next to nothing when I figure out what I've saved (bought a color laser printer for about $180, a 32" flat screen for about $220). When calculating the membership cost (I pay $55 p/y), I figured I save that on seaweed snacks alone when compared to Trader Joe's prices ($0.67 for the same amount $0.99 would buy at TJ's).

I will own my home in about 15 years. Refinanced last year for 20 years and I'm diligently making additional payments to have it paid off sooner
Don't eat out much period (once per week). Only once or twice a month at fast food. Like Ilse Berg, I usually pack a snack if I'm out for awhile.

Costco vs TJ? How silly. That is like comparing JC Penney with Sports Authority for buying shoes.
And even if there were a TJ in Colorado, it wouldn't be allowed to sell Wine. That's the law there.
